For UFC Undisputed 2010 Xbox 360, in order to unlock the XBox Live functionality of the game it requires a unique game code that it says is on the back of the game manual. If I wanted to keep the game, will the game manual and unique code be sent to me from Gamefly?

No. GameFly doesn't ship inserts or manuals. So the free content will never be included. Most of the time you can check the Game Marketplace for games, like UFC, and you'll be able to download that stuff (sometimes you gotta pay).
If you decide to keep the game you rented, GameFly will ship you the case for the game along with the manual. They've always been shipped to me in mint condition. They'll ship those for free too.
