If I chose to keep/buy the Batman came, does the code for catwoman come with the new case and instructions?

Yes, you get the case, manual, and anything else that was included in the original game. Which can include online passes and content codes.

slayer127 is right you get everything that was part of the standard retail package.
The only thing to keep in mind is that some unlock codes do expire. This is something that the publishers do and is out of the control of GameFly (or any one else for that matter). I do not know if the pass with B:AA is one that expires or not and if so what the window is. Generally when a pass expires (unused) it's a matter of contacting the publisher and getting a new one -- and from what I know, they usually give replacement codes for free. The last resort is to purchase a pass online (generally run ten bucks or so).
But really it shouldn't be an issue for a game so recent. And to repeat yes GameFly sends you everything in the retail box, no worries there.- 0
