Now, when exactly do the decisions you make during the game start affecting the story? Does the course of the plot stay the same until after you beat the final stage and Trisha comes in and talks a bit about the scene that took place after wards? Or does it change even earlier than that?

I have the 360 version so I don't believe there's a difference, but the endings depend on how far along you are along the good/bad meter (good, bad, or neutral) and how you answer the last four questions. For example, you can get 3 different endings if the meter is completely good but you answer the last 4 questions differently.
The questions you answer climbing the tower only affect the good/bad meter. I hope this helps!

it mostly depends on the moral choices you make like the cell phone text's and the questions in the elevator,the ending is determined by the answers you gave,i finished close to good and was extremely let down but that was because i put like 30 hours in to the game in about 3 days
