how do rank up in cod 3?

I don't think you can. I pretty sure that the whole ranking up thing started in cod4.

Yes, you don't rank up because you start over at every game. You start out as a private then a corporal and then a sergeant. I haven't played it in forever but i think you get small rewards for getting to corporal and sergeant.
