I kinda of think of it like this: When you reject the ways of Disney and Square Enix but somehow you are able to live in peace and happiness, the executives show up a la Indiana Jones and the Temple of Doom and perform a ritual that takes out your heart and then you become a Heartless. CA-LI-MAAAAAA...CAA-LIIIIII-MAAAAAAAAHH!!

I think it's kinda explained in the Ansem reports from the first game. I think it has to do with losing your heart to darkness. Sora becomes a heartless and the transformation is instant. I guess it could happen to anyone, suddenly. Strangely enough, Heartless aren't actually heartLESS, I mean, they had hearts of sorts which float out of them when they are killed. I think nobodies are supposed to be the 'true' beings without hearts, as they are literally supposed to be without emotion, and are the empty shell that gets left behind when a heartless is formed from a person's heart. Anyway, I'm not sure myself but I hope that helps.

Basically darkness overcomes your heart the heart gets trapped inside the heartless while the darkness squishes out all light and then dissapears when the heartless is distroyed the heart is released later into a helpng part making up [SPOILER ALERT] kingdom hearts a place where all nobodys get a heart when completed and actually become someone.The heartless after destroyed then transforming, if the hearts weak, into the tiny heartless like the ones that have a little weird hat and the shadows, the ones that are tiny and naked and so very weak.
The medium hearts are the ones that make up most heartless the magic, ghost, the fliers, all those and more they make up the majority.The fat people hearts go to the huge fat heartless that are really big. And the really strong hearts, like the princesses and Sora and Kiaria and Riku and the king are the bosses the things blocking you from the keyhole of course the other boss hearts are from other past key barriers and the REALLY strong hearted people.
