If at any time you loose anything that gamefly sends you, they'll send another if you need it, just click "My account" in the upper right hand corner of the screen then scroll down and click "report a shipping/purchase problem". Then click "report a RENTAL problem" Then click the game that you currently have the problem with, and since your issue is you lost the game sleeve then scroll all the way down and there'll be one called "lost the games sleeve" just click that then follow the instruction.
