When it says "at home" it doesn't mean it has arrived to your house. It just means that Gamefly has shipped the game to your house. Wait a few days and it will arrive.

If you rent the game from Gamefly, then you don't receive an online pass. If you buy it from Gamefly, then they will send you the online pass along with everything.

If you're trying to rent a game that came out this year, then you'll probably have to wait a few days for them to send it. I suggest trying to rent older games.
